Over and above the framework of personal data necessary to accomplish contractual arrangements, I hereby provide the controller with my contact details, which the controller shall maintain in a file with the data specified above. I hereby provide such contact details to the controller for the purposes of maintaining a marketing database of clients, and for the controller to send information in the future. I have the following rights with respect to the controller and my personal data: to request access to personal data, to correct personal data, to erase personal data or to restrict the processing of personal data, to object to the processing of personal data and to request the transfer of personal data. I have the right to file a petition to commence privacy proceedings under §100 of Act No. 18/2018 Coll. on Personal Data Protection. Personal data is not transmitted to any third country. The controller processes personal data continuously for the purposes of any future contact from the client and to respond to any additional health needs. The controller may, in the future, employ automated individual decision-making processes, including profiling, for marketing purposes.

I have the right at any time to revoke this consent to the processing of my personal data for the purposes of the controller’s marketing activities. Revocation of such consent has no retroactive effect on the lawfulness of the processing of personal data under such consent prior to its revocation.
